You probably have the device /dev/fd0u1722

For the main floppy:
# fdformat /dev/fd0u1722
# cat (floppy image) > /dev/fd0u1722

For the addons:
# fdformat /dev/fd0u1722
# cat srv.tgz > /dev/fd0u1722
(repeat for each addon desired)
