Floppy controller's problem, or floppy disk with bad blocks.
Another possible attack (sorry for this word):
1) using another computer, build the muLinux ISO image (roclone)
2) put the ISO image on a CD-ROM (using your favorite software)
3) boot the CDROM in you destination machine
4) run /bin/clone
